- Photo ID for each adult applying
- Proof of Michigan address (lease, bill or mail)
- Social Security numbers, or immigration documents, for each person applying
- Recent pay stubs or proof of income (last 30 days)
- Bank statements
- Rent or mortgage and utility bills
- Health insurance cards and medical bills, if any
- Child care provider details, if applying for CDC
- Any letters from MDHHS, and your MI Bridges login if you have one
